Admission Process

1. Planning for Admission

What to Prepare

  1. Personal Identification: Please bring at least two (2) valid government-issued photo IDs (e.g., driver's license, PRC license, passport).

  2. Payment Arrangements: Ensure to have your PhilHealth/HMO IDs and any HMO pre-authorization forms such as Letter of Guarantee (LOG) or Letter of Authority (LOA) or PhilHealth Member Data Record (MDR) if applicable. Bring your Senior and PWD IDs to avail of discounts.

  3. Medications and Prior Medical Records: Bring a list of your current medications, including names, dosages, and frequencies. Also have your prior medical records (e.g. lab results, x-ray films) ready, especially if you had a recent hospital confinement at another hospital.

  4. Emergency Contacts: Bring a list of emergency contacts/next of kin with their names and phone numbers.

2. Arriving at the Hospital

Parking

  1. Patients and visitors may park in the OGH Parking Building, which is accessible via Aurenina St. or Juanita De Leon St. Please see the picture below:

2. Parking Assistance: For parking assistance, please inform our security at the hospital main lobby.

3. Overnight Parking: If you plan to leave your car in the quadrangle parking area overnight, you will be asked to pay a fee of PHP 100 and provide contact details and basic information regarding your vehicle to hospital security.

4. Lost Ticket: A fee of PHP 500 will be charged for a lost parking ticket.

3. Admission Process

  1. Emergency Room (ER): Proceed straight to the ER and inform our ER staff that you are for admission. Our ER staff will inform your Attending Physician about your arrival, ask about your medical history, and perform a baseline physical exam.

  2. Admitting Section: You and/or your companion will then be directed to the Admitting Section just a few steps away from the ER, where our friendly staff will assist you with the admission process.

  3. Registration: You will be asked to complete a patient registration form where you will provide your personal data, payment details, and health information for us to process your hospital admission.

  4. Consent Forms: You may be required to sign consent forms for medical treatment or procedures. If you have questions or concerns, please do not hesitate to ask.

  5. PhilHealth/HMO Benefits: Our team will verify your insurance/HMO coverage and benefits. Please have your PhilHealth MDR and/or HMO pre-authorization forms ready. To avail of PhilHealth benefits, you must submit all requirements, including an accomplished original Claim Signature Form (CSF), prior to discharge. If you have any questions regarding PhilHealth or your HMO coverage, please let us know.

  6. Room Selection: Final room accommodation depends on the availability of the room type of choice. You may view our rooms through our photo gallery upon request from our admitting staff. 

  7. Payment and Initial Deposit: A reasonable amount is required as initial deposit for admission at our hospital. For non-urgent cases, the deposit is required to be settled upon admission. In case of emergency confinement, a deposit is required as soon as the patient is stable.

  8. Clearance for Admission and Initiation of Doctor’s Orders: Once you are cleared for admission, an admission slip will be given to you. Please show this slip to our ER staff so they may initiate your doctor’s admission orders. These orders may include administration of medications, intravenous insertions, and performance of certain procedures, which will be explained to you by our staff prior to initiation.

  9. Admission Kit and Transfer to Room: To ensure convenience during your stay, you will be given an Admission Kit containing essential items including, but not limited to, a face towel, toiletries, and eating utensils. Our hospital staff will also gladly assist you and your companion to your room.
