What personal data we may collect from you

In order to provide you with the best possible care, OGH may collect from you the following personal data, as may be applicable and necessary for various purposes:

  • Personal details such as name, age, gender, civil status, affiliations, parents’/spouse’s information;

  • Contact information such as addresses, email, mobile and telephone numbers, contact details of next of kin;

  • Health information such as physical, psychiatric and medical information;

  • Employment information such as government-issued IDs, numbers, position and functions;

  • Applicant information such as academic background, employment history, grades, course and academic standing.

why we collect your personal data

OGH may collect and process your personal data for the following purposes:

  1. To provide you with appropriate treatment and medical services;

  2. To coordinate care with physicians, nurses, allied medical personnel, other OGH employees, and referring institutions/healthcare personnel involved in your care;

  3. To confirm, bill and receive payment for health care services that we provided to you, which includes submission of your personal information to receive payment from Philhealth, your health maintenance organization (HMO), insurance company, or other parties that will pay for some or all of your health care. In cases of payment difficulties or non-payment, your personal and health information may be sent to social services for financial assistance or legal services for collection purposes;

  4. To comply with legal, regulatory, administrative or judicial requirements including but not limited to audit, reporting and transparency requirements of the Department of Health, PhilHealth, and other regulatory agencies;

  5. For other purposes not outlined above necessary for OGH to perform its obligations, exercise its rights, and conduct its functions as a medical institution.

how and when we collect your personal data

OGH generally collects your personal data upon initial interaction with any of our designated hospital personnel or at the onset of a service, or at the onset of a transaction, such as medical care, medical consultation, laboratory service, and health-related services, or when you provide feedback or responses to our surveys.

OGH collects your data physically through printed forms and other documents required by its medical units and administrative offices, or electronically through electronic systems, electronic platforms, e-forms, email, or electronic submission of information.

how we store, protect, retain, & dispose your data

OGH maintains the confidentiality, integrity and availability of your personal data as governed by our hospital’s privacy policy. Only authorized personnel will have access to your personal information in our files. Your personal data are stored in physical and electronic data processing systems managed by OGH. Physical records are generally stored in folders or envelopes in locked drawers or shelves while electronic records are stored in servers in the possession of OGH or in local storage.

All health information gathered and kept in medical records shall be retained solely for the fulfillment of the aforementioned purposes. With secure protocols in place, transmission, retention and disposal of your medical records are performed in accordance with Chapter III of the Data Privacy Act of 2012 and Rule V of its Implementing Rules and Regulations and local government standards.

when we may share your personal data

OGH may share your data with other healthcare providers in situations where there might be a transfer of care. There are also cases wherein the hospital will transmit your data to regulatory bodies, such as PhilHealth or the Department of Health, for data reporting purposes and to your HMOs or insurance companies for billing and payment purposes. OGH may also share your personal information when ordered by a court or a government authority such as in medico-legal cases.

Listed below are your rights with respect to your personal data:

1. Right to be informed

  • You have the right to be informed on how your personal and health information are being collected and processed. You have the right to be informed of the purposes for which they are being processed and the duration for which the information will be kept.

2. Right to access the information and data portability, subject to reasonable and lawful requirements:

  • You have the right to obtain a copy of your pertinent health information. The medical information available to you are the following:

    • Clinical Abstract/Discharge Summary

    • Record of Operation or Delivery

    • Operative Technique

    • Medical Certificate

    • Consent for Admission and Procedure

    • Laboratory and other diagnostic results

    To request for a copy of your medical records, proceed to the Medical Records Section or contact them at (02) 8826-7966 loc. 103. For laboratory and other diagnostic results, kindly proceed to or contact the diagnostic unit where the test was performed. Please note that OGH may charge a fee for the cost of providing copies to you.

3. Right to rectification or erasure of given information

  • If you believe that the personal information OGH has on file about you is incorrect or incomplete, you may ask us to correct the personal information in your records. For requests to make a correction to your records, proceed to the Medical Records Section or contact them at (02) 8826-7966 loc. 103. We may require additional documentation from you or your authorized representative as proof before processing your request.

4. Right to object on how we use the information

  • You have the right to request reasonable restrictions on certain uses or disclosures of your personal and health information. In most cases, we are not required to agree to your requested restriction. However, if we do agree, we will comply with your request unless the information is needed to provide you emergency treatment or comply with the law.

    Some examples of restriction requests that the Hospital cannot honor include:

    • Requests to restrict healthcare personnel from accessing your medical information.

    • Requests restricting the hospital from giving your name to an insurance company that will be asked to pay a portion of your bill.

    • Request restricting the hospital from reporting your identity and condition to an agency or organization where the hospital is required by law to do so.

5. Right to file a complaint

  • OGH’s Data Protection and Customer Service Offices are always available to assist you in resolving your concerns.
